- Why does it have to be compounded?
- Commercial naltrexone is a 50 mg tablet. The low-dose protocol uses roughly a tenth of that, which requires a compounding pharmacy to prepare accurately.
- The vivid dreams are intense. Should I stop?
- Usually not — they typically settle within two weeks. If they don't, we drop back to the previous dose and titrate more slowly.
- Can I take it with opioid painkillers?
- No. Naltrexone blocks opioid receptors and can precipitate withdrawal. Tell your clinician about any opioid use during intake.
Vivid dreams and mild sleep disruption in the first one to two weeks are by far the most common, and usually resolve on their own. Headache and transient nausea occur less often. If you take any opioid medication, LDN is not an option.

What is Low Dose Naltrexone?
At standard doses naltrexone blocks opioid receptors continuously. At roughly a tenth of that dose, the blockade is brief.
That transient blockade appears to trigger a compensatory increase in endogenous endorphin production once it lifts.
