Do I need diabetes to take it?
No. We prescribe it off-label for insulin resistance and as part of longevity protocols. Your labs and history determine whether it's appropriate.
Will it interfere with building muscle?
Some studies suggest metformin can blunt certain training adaptations, particularly at higher doses. If hypertrophy is a primary goal, discuss dose and timing with your clinician.
Why extended release?
Almost entirely tolerability. The extended-release form causes considerably less GI upset at the same total daily dose.

GI upset — loose stools, bloating and cramping — is common in the first weeks and is substantially reduced by using the extended-release form and taking it with food. Persistent symptoms usually mean titrating more slowly.

Who this is not for

  • Requires adequate kidney function; we check before prescribing
  • Long-term use can lower B12 — we monitor and supplement
  • May blunt some training adaptations at higher doses
  • Must be held before procedures using iodinated contrast

What is Metformin?

(met-FOR-min)

Metformin reduces hepatic glucose production — the liver's contribution to your fasting blood sugar — which is its primary and best-established action.

It activates AMPK, a cellular energy sensor that also mediates part of the benefit of exercise and caloric restriction.

How Metformin works

  1. Week 1

500 mg extended release with food. GI symptoms are most likely now and are the reason we start low.

  2. 30 Days

First increase if tolerated. Most patients report the initial GI effects have already faded.

  3. 3 Months

Repeat metabolic panel. Fasting insulin and HbA1c are the markers we expect to have moved.

  4. 6 Months

Kidney function and B12 checked, dose reviewed against results rather than habit.

  5. 1 Year and Beyond

Annual monitoring continues. Metformin is a long-horizon intervention and we treat it that way.
