This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully. It is provided in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
Uses and disclosures without your authorisation
- Treatment: sharing your information with clinicians, pharmacies and laboratories involved in your care.
- Payment: processing payment for services rendered.
- Health care operations: quality review, clinician oversight and business management.
- As required by law: including public health reporting and judicial orders.
Uses requiring your written authorisation
Most uses and disclosures of psychotherapy notes, uses for marketing purposes, and any sale of protected health information require your written authorisation. You may revoke an authorisation in writing at any time.
Your rights
- Inspect and obtain a copy of your health record, in electronic form where we maintain it electronically.
- Request an amendment to information you believe is incorrect or incomplete.
- Request an accounting of certain disclosures we have made.
- Request restrictions on certain uses and disclosures.
- Request confidential communication by an alternative means or at an alternative address.
- Receive notification of a breach of unsecured protected health information.
- Obtain a paper copy of this notice on request.
Our obligations
We are required by law to maintain the privacy of protected health information, to provide you with this notice of our legal duties and privacy practices, and to notify you following a breach of unsecured protected health information. We are required to abide by the terms of the notice currently in effect.
Complaints
If you believe your privacy rights have been violated you may complain to us using the contact page, or to the Secretary of the US Department of Health and Human Services. You will not be retaliated against for filing a complaint.
